LIGHTING SUBMITTAL REQUEST

Location changes, fixture changes/ substitutions, or any change that may affect the outcome of the lighting design must be approved by the lighting designers in writing utilizing this form prior to implementation. Any changes, substitutions, or omissions made without prior written approval shall be the responsibility of the electrical installer to rectify to the satisfaction of the lighting designers and client.
